Cracked Tooth Syndrome: Why Teeth Can Hurt Without a Visible Cavity

I often see patients who come in frustrated because they feel sharp pain when biting or chewing, yet they’ve been told there’s no cavity and their x-rays look normal. In many of these cases, the cause is a condition called cracked tooth syndrome.
What Is Cracked Tooth Syndrome?
Cracked tooth syndrome occurs when a tooth develops very small cracks that are often impossible to see with the naked eye or on standard dental x-rays. These cracks usually develop over time from everyday chewing forces, teeth grinding, trauma, or from having large fillings that weaken the remaining tooth structure.
Even though the tooth may look intact, these tiny cracks can still cause significant discomfort.

Common Symptoms I See in Patients
Patients with cracked tooth syndrome often describe symptoms that come and go, such as:
Sharp pain when biting down or releasing pressure
Sensitivity to cold or hot foods
Pain that may disappear for days or weeks and then suddenly return
Why X-Rays Often Look Normal
One of the most confusing aspects for patients is that cracks frequently do not show up on X-rays. When you bite down, the crack can open slightly and irritate the nerve inside the tooth, causing pain. Once the pressure is released, the crack closes again, making it difficult to detect without a careful clinical exam.
Why Teeth With Large Fillings Are at Higher Risk
I commonly see cracked tooth syndrome in teeth with large fillings. When a significant portion of natural tooth structure is replaced with a filling, the remaining enamel becomes weaker and more likely to flex and crack under chewing forces.
How I Treat Cracked Teeth
Treatment depends on how deep the crack extends. In early cases, placing a crown can protect the tooth, stop the crack from spreading, and relieve symptoms. If the crack has reached the nerve, root canal treatment may be needed before the crown. Early diagnosis is key to preventing a more serious fracture or tooth loss.
